Floriamed, LLC (“Floriamed”, “we”, “our”) owns and operates the website, floriamed.com. So Floriamed is the entity that collects data about you. We are serious about our responsibility to protect your privacy. Our dedication to patient advocacy is the source of our commitment to protect your privacy and best interests.
The floriamed.com website collects information about you in two ways. One way is when you enter information into a form and submit it. The other way is by using cookies. A cookie is a small data file that websites store inside your browser for later use.
Who Receives the Data Submitted In a Form?
Who receives the data submitted in a form depends on what you want. If you want a visit with a healthcare provider, the data you submit will go directly to either your healthcare provider or a payment processor as appropriate.
When you have a healthcare visit using floriamed.com, your healthcare provider is working for CareProviders Advanced Practice Nursing PLLC (“CareProviders”). So CareProviders is the entity that receives the healthcare information you submit. And your healthcare provider is the person who receives that information. The payment data you submit goes directly to Stripe, the payment processing company.
However, if you submit a form to contact us for any other reason besides a healthcare visit, the data will go directly to administration at Floriamed/CareProviders for placement.
Regardless of why you submit a form using floriamed.com, no data received is ever stored on floriamed.com. And nobody can view any of the submitted data using floriamed.com.
About Healthcare information Submitted By Form
For safety, healthcare providers require a minimum of information before beginning an online visit. This is personal information that identifies you. Federal HIPAA regulations classify the data you submit as “Protected Health Information” or “PHI” (see more on this in Patient Privacy and HIPAA and Notice of Privacy Practices). So your provider handles the data you submit in accordance with HIPAA law.
HIPAA law gives you privacy rights and helps protect your privacy. It also gives you some choices and options about who can use your PHI and how they can use it. Once your healthcare provider receives the PHI you submitted, they transfer it to an electronic medical record that they create for you. This medical record is where your PHI is stored. Medical records are likewise subject to HIPAA law and the security standards dictated by it.
Proper healthcare requires medical records. Your healthcare provider will use your medical record to identify you in the event of future visits. During every visit you continue to share PHI with your provider as you speak. Your single medical record is where your provider records and stores the results of all visits you have using floriamed.com. All your PHI is stored in one place, namely, your medical record.
About Payment Information Submitted By Form
The payment information you submit moves directly from your browser to the servers at Stripe for processing and storage. Stripe uses the submitted data to process your transaction, verify identity, enhance security, and detect and prevent fraud.
The payment information you submit to Stripe is shared with parties in connection with your transaction. This sharing is necessary to enable the transaction. One of the parties with which Stripe shares this data is CareProviders. CareProviders may use this data to confirm or cancel payments, or to help resolve payment problems or disputes.
